Injection site reactions and injection-site reactions: I was getting red, itchy welts at every injection site that lasted 3-4 days.

For those on injection-site reactions — a practical guide to injection site rotation. I use a 4-site system:

  • Week 1: Right abdomen (2 inches from navel)
  • Week 2: Left abdomen
  • Week 3: Right thigh (outer, middle third)
  • Week 4: Left thigh

Then repeat. This gives each site 4 weeks to recover. I mark it in my phone calendar so I never inject the same area two weeks in a row.

Same technique for eight months and the site reactions started in month nine, which rules out most of what I assumed the cause was.

Has anyone on injection-site reactions tried splitting their dose across two injection sites? My provider mentioned some patients do this at higher doses (2.4mg sema or 15mg tirz) to reduce injection site reactions.

I'm about to move to 15mg and want to minimize the lump that forms at the injection site.

Helpful tip for injection-site reactions needle anxiety: ice the injection site for 30 seconds before injecting. You'll barely feel the 30g needle go in. I went from dreading injection day to not even flinching.

Also, don't watch the needle enter your skin. Look away, take a deep breath, and it's over in seconds.

One thing that is still open after Dr.SleepRoch’s answer:

Did your prescriber agree with that reading, and if not what was their objection?

Side effect timeline for injection-site reactions based on my experience and tracking:

Side EffectOnsetPeakResolution
NauseaDay 1-2Week 1-2Week 3-4
ConstipationWeek 1Week 2-3Ongoing (managed)
FatigueDay 2-3Week 1Week 2-3
HeadacheDay 1Day 2-3Week 1
Injection siteImmediateDay 1Day 2-3

Most side effects are transient and self-limiting. The key is pushing through the first month — it gets dramatically better.
