Started this for PCOS-related insulin resistance, and the thing nobody prepared me for was that my cycle came back within two months.

What I am after is what the washout thinking is before trying to conceive, and why nobody mentions that fertility often increases as a side effect.

If the honest answer is that nobody knows, that is a useful answer and I would rather have it.

I understand the enthusiasm, but "everyone" is too broad. Contraindications exist: personal/family history of MTC or MEN2, history of pancreatitis, pregnancy/breastfeeding, and certain GI conditions.

Beyond contraindications, clinical judgment matters. A 22-year-old with BMI 28 and no comorbidities has a different risk-benefit calculation than a 55-year-old with BMI 38 and metabolic syndrome.

Let's advocate for ACCESS without advocating for UNIVERSAL use. They're different things.

Fair, and it stops being fair at the extremes. The middle of the range is well understood; the ends are where the honest answer is that nobody knows.

Adding the clinical framing, because it changes how the question reads. It is worth asking what the claim would look like if it were false. If nothing would look different, it is not a claim about the world and no amount of discussion will settle it.
