Clinical perspective, offered as context rather than as advice. Start from the measurement rather than the conclusion. Almost every disagreement here turns out to be two people measuring different things and comparing the numbers anyway.

Practical: whatever you change, write down the date and the reason.

There is a second half to this that has not been said yet. The version of this that has an answer is narrower than the version being asked. Narrow it and it becomes tractable; leave it broad and the thread will produce nine confident and incompatible replies.

If somebody has the primary source to hand I would rather cite it than paraphrase it.

Adding the numbers, since they settle part of this. The dose-response is real but shallow at the top. Across STEP 1 and STEP 4 the gap between 1.7mg and 2.4mg is a couple of percentage points of body weight on average, and the average is carrying a wide spread — plenty of people at 1.7mg sit above the 2.4mg mean. If a dose is working and tolerable, "working" is the relevant variable, not "maximal".

One thing that is still open after dan_philly’s answer:

How much of the between-person variation is pharmacokinetic and how much is just adherence measured badly?

Moderator note: a post naming a supplier has been edited. Discuss suppliers in the review sections, not here. Carry on.
