Got my tirzepatide delivery today from a vendor I won't name yet (want to give them a chance to respond first). The package had NO cold pack, NO insulated liner, just a vial in a padded mailer. It's been sitting in my mailbox in 85°F weather for who knows how long. The reconstituted solution is supposed to be clear but this looks slightly cloudy. Is the product ruined? Am I overthinking this? This was a $270 vial and I'm really frustrated. I've already emailed the vendor with photos. What should I expect them to do here?

Do NOT use a cloudy peptide solution. Full stop. Cloudiness can indicate protein aggregation or bacterial contamination, neither of which you want to inject. Regarding the temperature exposure — peptides like tirzepatide are relatively stable in lyophilized (powder) form and can tolerate short periods at room temperature. However, 85°F for an extended period is pushing it, and if it was already reconstituted that's a much bigger problem. Was it lyophilized powder or pre-mixed liquid when it arrived?

It arrived as lyophilized powder. I reconstituted it myself after it arrived. The cloudiness appeared after reconstitution — maybe 10 minutes after mixing. Normally when I reconstitute it goes perfectly clear within a minute or two.

The fact that it was lyophilized is somewhat better news — lyo powder is more heat-stable than reconstituted solution. But the cloudiness after reconstitution suggests the peptide may have degraded or the lyophilization cake was compromised. A few questions: 1. Did the powder cake look normal when you opened it? (Should be a white/off-white solid puck) 2. What did you reconstitute with and how much? 3. Did you swirl gently or shake vigorously? (Shaking can cause foaming that looks cloudy) If you swirled gently and it's still cloudy after 30 minutes, I would not use it.

1. The cake looked slightly collapsed/crumbly compared to my previous orders from another vendor where it was a solid puck 2. Bacteriostatic water, 2mL as directed 3. I always swirl gently along the side of the vial, never shake So yeah, the cake was already looking off before I even reconstituted. This is definitely a product quality issue compounded by the shipping failure.
